Employees are reminded that they have a duty to ensure that the company's health and safety policy is observed and in particular they are required:

In turn, all levels of management and all supervisors are responsible for carrying out those health and safety duties placed on them. The managing director will ensure that the safety plan includes sufficient resources for the successful implementation of the health and safety management policy.

Notes:
To calculate the Accident Frequency Rate, take the number of reportable accidents in the year and multiply the figure by one hundred thousand. Divide the resultant figure by the total number of employee hours.
i.e. 14 accidents times 100000 = 1400000, divided by a total hours 5100000 for all employees = an AFR of .275
To calculate the Accident Severity Rate, take the number of days lost and divide by the number of accidents. This gives an average number of days lost per accident.
i.e. Total days lost = 76 divided by 14 = 5.42.
Days lost not recorded for the years 1997 to 2000 (Previous years data available upon request)
